Wish to send a card, letter or package to a patient in the hospital? It’s important to make sure it’s labeled correctly. Please use the following format:

Name (Patient)
Floor or Dept.
Hospital Name
Hospital Address
City, State and Zip Code

Example:
Jane Doe (Patient)
5th Floor OR Birthing Center
Essentia Health-St. Mary's Medical Center
407 E 3rd St.
Duluth, MN 55805

Please write (patient) in parentheses next to the name.

To find the hospital name, address or phone number, use our Locations & Facilities finder. Search by facility type and choose “hospital.” If you don’t know the floor number or department name, you can call the hospital.
